THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

We are committed to protecting the privacy of your health information (“Protected Health Information” or “PHI”). This Notice explains how we may use and share your PHI and your rights regarding it. We are required by law to keep your PHI private, to give you this Notice, and to follow its terms.

How we may use and disclose your health information

Treatment, payment, and health care operations

We use your PHI to provide and coordinate your care; to document services and prepare a superbill or receipt so you can seek reimbursement (we do not bill insurance directly); and to run our practice, such as quality review, scheduling, and appointment reminders.

As required or permitted by law

We may use or disclose your PHI when required by law; to public health authorities; to report suspected abuse, neglect, or domestic violence; for health oversight; in judicial or law-enforcement matters as the law allows; to coroners or medical examiners; to avert a serious threat to health or safety; for workers’ compensation; and for specialized government functions.

Business associates

We may share PHI with vendors who perform services for us — such as our practice-management, telehealth, and AI-assisted documentation vendors — only under written agreements requiring them to protect your information.

Uses that require your written authorization

Most uses and disclosures of psychotherapy notes (if any), uses for marketing, and any sale of PHI require your written authorization, which you may revoke in writing at any time.

Your rights

  • Inspect and copy your health record (a reasonable, cost-based fee may apply); request an electronic copy or that a copy be sent to a person you designate.

  • Request corrections or amendments; request an accounting of certain disclosures.

  • Request restrictions on certain uses and disclosures. If you pay in full out of pocket for a service, you may ask us not to share information about it with a health plan, and we will honor that.

  • Request confidential communications by alternative means or location.

  • Receive notice of a breach of your unsecured PHI; receive a paper copy of this Notice on request.

Changes, complaints & contact

We may change this Notice and make the new terms apply to all PHI we maintain; the current Notice will be posted at our office and at drlyndsey.com. If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights (www.hhs.gov/ocr/). We will not retaliate against you for filing a complaint.
